New Patient Registration
If you live within our practice boundary, you can register as a patient with us. To check your address please use our postcode checker. Please note we only accept new patients from addresses within our boundary area.
To register with us, you will need to complete both the Purple New Patient GMS1 form and the New Patient Questionnaire. This can be done by downloading and completing the documents below and then bringing them into the practice. Alternatively you can collect printed copies from the Cinderford Medical Centre.
The registration process can take a few weeks to complete and all your information to be transferred to us from your previous practice. If you are on regular medication please make sure you have had a recent prescription from your current GP before you start the new registration process. We will be able to offer urgent care from the time you submit the forms, but may not have sight of your medication history until the electronic transfer of your records has been completed.

Once you have completed the forms, please bring these into the Medical Centre with photo ID and proof of address so that we can check you live within the boundary.
If you wish to have online access to book appointments, order repeat medication and view your medical record, you will need to bring two forms of ID with you, one of which should be photographic.
It is a requirement that any changes in personal details such as name, address, contact telephone numbers etc are communicated to the practice promptly. You can let us know via our Change of Personal Details Form.
Please read our full Practice Registration Policy before completing this form.
