Consent to immunisation
Consent for immunisation can only be given by the child’s mother or father (if the father has parental responsibility). As we cannot routinely verify parental responsibility at appointments, we ask that if someone without parental responsibility brings your child, the mother is available by phone at the appointment time to provide consent.

Declining vaccinations
We are required by the Child Health Department to ensure children are vaccinated according to the UK schedule and we will continue to send invitations. If you wish to decline any vaccinations, you MUST complete the Refusal Form below and return it to the surgery as soon as possible. The completed form will be sent to the Child Health Department so they can update their records.
